For the 2024, CSI Volleyball camps during the dates set forth on this application. In consideration of your acceptance of this application, I, as the parent or guardian of my child hereby release the College of Southern Idaho, the Participating School District and all their employees and agents from all claims on account of any injuries which may be sustained by my child while attending the CSI Volleyball Camps.
